Teams Traveling under USYS (from inside Region III)
· Team must have five (5) copies of the Official USYS team roster with all jersey numbers noted · All states in Region 3 should check with their own state office to see what is required for your team to travel. Please be prepared to have an approved Permission to Travel form approved by your state office or any other form that is required.
· Team must have medical release forms properly signed and notarized – forms that meet your home state requirements
will be accepted
· Team must have signed, laminated player and coach cards with pictures
· Team must have a completed Emergency Contact Form · TN teams with guest players must complete, submit and print out the online “TN Guest Player Permit Form” at: https://ssl.hammersoccer.com/tennessee/travel/GuestPlayerForm.asp · Team must input roster online by Monday, May 1st, 2010

Teams Traveling under U.S. Club Soccer
· Team must have five (5) copies of the Official UClub team roster with all jersey numbers noted
· Team must have medical release forms properly signed
· Team must have laminated player and coach cards with pictures
· Team must have a completed Emergency Contact Form
· Team must input roster online by Monday, May 1st, 2010
Foreign Teams
· Players must present passports at registration, or if from a national that the United States does not require a passport,
proof of entry into the United States that is required by the United States.
· Teams must have a completed form from its Provincial or National Association approving the team’s participation in the
tournament.
· Team must have five (5) copies of the team roster with all jersey numbers noted
· Team must have medical release forms properly signed
· Team must have laminated player and coach cards with pictures
· Team must have a completed Emergency Contact Form
· Team must input roster online by Monday, May 1st, 2010
All Players
The shirt number of each player must be the same as the player’s shirt number on the roster. If the numbers are not the same, the referee is not to allow the player to take part in the match until the numbers are the same. Both teams must bring two sets of uniforms to each game, the home team will wear white, and the team not in compliance will be required to change if there is a conflict.
Guest Players
· Team must provide a guest player permission form if required by the team’s resident state
· Team must have a medical release form properly signed and notarized
· Team must have a signed, laminated player card with picture
