Field request form is available on the link to the left.

1. Send all field and referee requests to John O’Brien (executivevp@brentwoodsoccerclub.com; 615-579-6105). John will reply with an e-mail confirmation back to you and Terry Hendrixson (Referee Assignor).

2. In the rare instance when you are requesting a field without the need for referees (i.e. scrimmage) you must notify John.

3. If your team is highlighted in green on the schedule (which will be updated and sent to you at least once a week) you are confirmed for both fields and referees. You have provided game start times and opponents. If you are highlighted in green and don’t need referees it is your responsibility to notify John.

4. If your team is bolded in yellow on the schedule, you are confirmed for the field but you have not yet requested or been assigned referees. You have provided only game start times (no opponents).

5. If your team is on the schedule (but not highlighted) your team is only tentatively scheduled, but with no degree of certainty and no referees assigned. You have only requested a field with a beginning use and ending use time.

6. It is your responsibility to review the schedule to ensure that your requests are accurately depicted, and if not, notify John.

7. Tentative field requests will be reviewed 10 days prior. If the field request has not been confirmed with the form below the field request will be removed from the schedule.

8. You will receive an e-mail from John every Thursday, reflecting Terry’s referee schedule for the weekend. This is another chance for you to review and confirm your referee needs.

9. You must call (not e-mail) Terry (cell: 615-948-2474) (not John) to cancel referees at least 24 hours in advance or your team will be charged the referee fees.

10. Teams canceling games (not due to inclimate weather) after referee assignments have been made will be responsible for the referee assignor fees ($5.00 per game).

11. If you do not need fields that you have tentatively reserved, please notify John so the fields can be released to others.

12. The field request form below must be completed fully to ensure referees are assigned to your games. Game times and opponents MUST be provided.

13. Referees must be requested at least 1 week prior to games.
