AWI-REGISTRATION REQUIREMENTS
All Players
| | The shirt number of each player must be the same as the player's shirt number on the roster. If the numbers are not the same, the referee is not to allow the player to take part in the match until the numbers are the same.
|
Teams Traveling under USYS (from inside Region III)
| | Team must have five (5) copies of the team roster with all jersey numbers noted
|
| | Team must have medical release forms properly signed and notarized - forms that meet your home state requirements will be accepted
|
| | Team must have signed, laminated player cards with pictures
|
| | Team must have a completed Emergency Contact Form (click here)
|
Teams Traveling under USYS (from outside of Region III)
| | Team must have five (5) copies of the team roster with all jersey numbers noted
|
| | Team must have a "Permission to Travel" form approved by their state association
|
| | Team must have medical release forms properly signed and notarized - forms that meet your home state requirements will be accepted
|
| | Team must have signed, laminated player cards with pictures
|
| | Team must have a completed Emergency Contact Form (click here)
|
Teams Traveling under U.S. Club Soccer
| | Team must have five (5) copies of the team roster with all jersey numbers noted
|
| | Team must have medical release forms properly signed
|
| | Team must have laminated player cards with pictures
|
| | Team must have a completed Emergency Contact Form (click here)
|
Foreign Teams
| | Players must present passports at registration, or if from a nation that the United States does not require a passport, proof of entry into the United States that is required by the United States
|
| | Teams must have a completed form from its Provincial or National Association approving the team's participation in the tournament
|
| | Team must have five (5) copies of the team roster with all jersey numbers noted
|
| | Team must have medical release forms properly signed
|
| | Team must have laminated player cards with pictures
|
| | Team must have a completed Emergency Contact Form (click here)
|
Guest Players
| | Team must provide a guest player permission form if required by the team's resident state
|
| | Team must have a medical release form properly signed and notarized
|
| | Team must have a signed, laminated player card with picture
|
| | There is a maximum number of 3 guest players per team permitted
|
